1. BOOKING APPOINTMENTS

• Book online, via email, or by phone

• You will receive a confirmation via SMS or email once your appointment is scheduled

 

REFERRAL REQUIREMENTS:

• No GP referral is required for pelvic floor or musculoskeletal consultations

• If you have any medical reports, scans, or GP referrals, please bring them to your appointment to assist your care

 

TELEHEALTH:

• Telehealth sessions are available for eligible clients

• You will receive a secure video link and instructions prior to your appointment

 

2. ARRIVAL & CONSULTATION TIME

• Arrive a few minutes early to complete any required forms

• If you are running late, please contact us. Late arrival may reduce your consultation time

 

3. CANCELLATION & NO-SHOW POLICY

• At Functional Movement Physio, each appointment time is thoughtfully set aside for you. To help us provide dedicated care to everyone, we kindly ask for at least 24 hours’ notice if you need to cancel or reschedule your appointment.

• Appointments cancelled with less than 24 hours’ notice may result in forfeiture of your deposit. Missed appointments without notice may incur the full consultation fee.

• We understand that unexpected situations can arise. If you are unwell or experiencing unforeseen circumstances, please contact us as soon as possible and we will do our best to assist.

• Thank you for respecting our time and the time of others who may be waiting for an appointment.

 

4. PATIENT RIGHTS & RESPONSIBILITIES
